Insurance Billing Information

A health insurance policy is a contract between the policyholder and the insurance carrier. If you have dental insurance, we want to help you receive your maximum allowable benefits. However, dental insurance does not relieve the patient of financial responsibility for services rendered. Our fees are not always the same as the amounts reimbursed under private or commercial dental plans. It is your responsibility to contact your insurance company prior to your appointment to clarify your own benefit levels, any referral requirements, deductibles, copays, etc. as you are primarily responsible for all charges.

As a courtesy, we are happy to submit to your primary insurance carrier so long as we are provided with complete and accurate insurance information. Please bring your insurance card and/or claim form with you to insure proper filing of your claim.

While it is our intent to provide you with as accurate as possible financial information, this office cannot give an exact estimate of insurance benefits since we do not have access to all insurance company records. Should a balance remain following payment by your insurance carrier, you will be billed for this amount. If your insurance carrier fails to pay within 45 days, you will be responsible for the full amount.
